• a filtered overview of incoming business payments for the last 6 months (for income calculation)
• full profit and loss statement for 2025 and 2024
• full balance sheet for 2025 and 2024
• property details (address, unit number)
• contact for the valuation (full name, e-mail and phone)
• cost estimate for the reconstruction/construction (attached, please send it back signed)
• scan of your current loan agreement (if we are arranging a refinancing)
• number of properties owned in the Czech Republic and abroad
• completed and signed sworn statement on the number of properties owned (form attached)
• completed and signed sworn statement on income for the last year (2025) (form attached)

• net floor area of the apartment (in m²)
• apartment layout (e.g. 1+kk, 2+1, 3+kk, etc.)
• building material (brick / panel)
• building age (up to 15 years inclusive / over 15 years)
• technical condition of the building — choose one of the following:
– new build (within 5 years of completion)
– excellently maintained (within 5 years of full renovation or within 10 years of completion)
– well maintained (normal wear, good safety, good quality)
– poorly maintained, partial renovation needed (slight reduction in safety and quality, defects beyond normal wear)
– poorly maintained, full renovation needed (substantial reduction in safety and quality, faults and serious defects)
• to send: 1× interior photo + 1× exterior photo of the building with the house number
